How to reduce appointment no-shows: what the research says works

The most reliable way to reduce appointment no-shows is a text reminder before the appointment that lets the customer confirm or reschedule by replying. A 2016 meta-analysis of 21 studies found reminders cut the no-show rate from 21% to 15%. Pair it with an easy reschedule link and a way to refill the freed-up slot.
What the research actually found
Most of the good data comes from healthcare, because clinics track attendance closely and publish the results.
The biggest study is a 2016 systematic review in BMJ Open by Robotham and colleagues at King's College London. They pooled 21 studies with more than 16,000 patients. People who got a digital reminder were 23% more likely to attend. The no-show rate dropped from 21% to 15%.
A randomized trial in a US pediatric clinic, published in the International Journal of Pediatrics the same year, found a bigger gap: 23.5% no-shows for families who got text reminders against 38.1% for those who didn't.
Six percentage points doesn't sound dramatic. Run the math on your own book, though. A chair with 30 appointments a week and a 20% no-show rate loses six slots. Bring that down to 15% and you get roughly one and a half slots back every week, from something you set up once.
Does this carry over to a salon or a garage? The studies are clinical, so nobody's proven it for oil changes. But the reason reminders work is plain forgetting, and people forget a haircut as easily as a checkup.
Why people don't show up
It's rarely rudeness. Usually it's one of these:
- They forgot. The appointment was booked three weeks ago on a phone call.
- Something came up and cancelling felt like a hassle (call during business hours, sit on hold).
- They booked somewhere else and never told you.
- They weren't sure the booking went through in the first place.
A reminder fixes the first one. The other three need the reminder to make cancelling and rescheduling easy. That's the part most businesses skip.
How to set up reminders that cut no-shows
- Send a confirmation the moment they book. Date, time, address and a reschedule link. This kills the "did it go through?" doubt.
- Send a reminder a day or two before. Ask them to reply C to confirm or R to reschedule. A reply gives you a real signal instead of hope.
- Send a short same-day nudge for early or long appointments. One line with the time and the address. Skip it for 10-minute walk-ins.
- Make rescheduling one tap. Link straight to online booking so they can move it themselves at 10pm instead of ghosting you.
- Refill the slot. When someone cancels, text your waitlist. A cancellation you hear about the day before is a slot you can still sell.
- Track it monthly. No-shows, late cancels and refilled slots. If the number isn't moving, change the timing or the wording.
What goes wrong: reminders sent from an unregistered number quietly fail to deliver. If your provider texts from a local number, it needs A2P 10DLC registration first.
What should a reminder text say?
Short. Business name first, so it doesn't look like spam. The time. One clear action. An opt-out. Something like:
"Hi Maria, it's Green Hills Dental. Reminder: cleaning tomorrow, Thu 2:30pm. Reply C to confirm or R to reschedule. Reply STOP to opt out."
Leave out the paragraph about your cancellation policy. Put that in the booking confirmation, once.
Should you charge a deposit or no-show fee?
For long or expensive appointments (a color treatment, a two-hour repair slot, a new-patient exam), a deposit taken at booking changes behavior more than any reminder. People show up for things they've already paid for.
For short, routine visits, a fee mostly annoys regulars. Start with reminders and easy rescheduling, measure for a month, and only add deposits to the slots that keep going empty.
Reminders alone won't fix it if booking is hard
If the only way to reschedule is calling during the day, people won't. That's why appointment reminders and online booking work best as a pair: the reminder prompts the decision, and the booking link lets them act on it right away.
